Everything that feeds the Decodemeno AI on the peri/menopause transition. 8 books, 16 pivotal clinical trials, 14 official guidelines from 2022-2026, 6 major regulatory decisions, 5 validated scales. It is all listed here, in descending order of authority, because we do not make anything up.
The Decodemeno AI reads every one of these sources before writing your perimenopause report. This page exists so you can check the work, share it with your doctor, or go deeper. Understand before you treat.
Guidelines & professional societies (2022-2026)
The institutional foundation for managing perimenopause and menopause, and the highest authority on this page. What NICE, the Menopause Society, the BMS, ESHRE, NOGG, USPSTF, AHA/ACC, ESC, EAS, the Endocrine Society, and the AUA recommend today. This is what dictates clinical practice.
NICE NG23 (United Kingdom)
The AI's reference on clinical diagnosis after age 45, the preference for the transdermal route, the Body Identical HRT position, and the hierarchy of non-hormonal treatments.
www.nice.org.uk/guidance/ng23The Menopause Society (formerly NAMS) (United States)
The North American framework on individualizing HRT, the timing hypothesis, and the absence of any arbitrary maximum duration.
www.menopause.orgThe Menopause Society (United States)
The hierarchy of non-hormonal options for hot flashes (CBT, paroxetine, venlafaxine, gabapentin, fezolinetant). Black cohosh, phytoestrogens, clonidine, and pregabalin are set aside.
www.menopause.orgBritish Menopause Society (United Kingdom)
Practical protocols on testosterone in women (HSDD, doses, monitoring), progestogens, and HRT monitoring schedules. A hands-on source for clinical tools in the UK.
thebms.org.ukInternational Menopause Society (international)
An international framework that consolidates the regional guidelines on HRT, bone health, and cardiometabolic health.
www.imsociety.orgESHRE (Europe)
The European position on the limited role of AMH and FSH after age 45, and their targeted usefulness in women under 40 for diagnosing primary ovarian insufficiency.
www.eshre.euNOGG 2024 (United Kingdom)
First-line treatment for postmenopausal osteoporosis, most often weekly alendronate, with anabolics (romosozumab) when the T-score is below -2.5 along with a fracture.
www.nogg.org.ukUSPSTF 2025 (United States)
DEXA screening for all women starting at age 65, earlier if there are risk factors (family history, low BMI, smoking, corticosteroids, early menopause).
www.uspreventiveservicestaskforce.orgISCD 2023 (international)
The reference tool the AI cites whenever a T-score comes up, covering central DXA, interpretation, and follow-up.
iscd.orgAHA / ACC 2026 (United States)
A modern lipid workup, Lp(a) at least once in a lifetime, ApoB measured selectively when the metabolic profile is at risk, CAC for reclassification, and the PREVENT-ASCVD equations.
www.acc.orgEAS (Europe)
An Lp(a) threshold of 50 mg/dL (125 nmol/L) as a cardiovascular risk enhancer. Used to interpret an elevated result.
www.eas-society.orgESC 2024 (Europe)
A new elevated-BP category between 120-139 / 70-89 mmHg, with a systolic target of 120-129 mmHg when treatment is tolerated. Worth knowing it diverges from ESH 2023.
www.escardio.orgEndocrine Society 2024 (United States)
Dropping a universal 25-OH-D target and routine screening. The priority is to avoid outright deficiency and to supplement at-risk profiles pragmatically.
www.endocrine.orgEndocrine Society 2025 (United States)
A hard rule. Never stop denosumab without a bisphosphonate to take over within the following 6 months. The source for the warning carried in section 6 of the report.
www.endocrine.orgAUA / SUFU / AUGS 2025 (United States)
A major shift, local vaginal estrogen can be discussed with breast-cancer survivors after moisturizers and lubricants have failed, as a shared decision. Extra caution on aromatase inhibitors.
www.auanet.orgMajor regulatory decisions (2023-2026)
FDA, MHRA, EMA. The decisions that changed what can be prescribed, under what conditions, with what monitoring. This is what actually changes the pharmacy available to women.
FDA, November 2025
What it means in practice. The FDA commissioner calls it a scientific correction. The fear inherited from the 2002 WHI extrapolation is no longer a scientifically defensible position for most profiles.
www.fda.govFDA, October 2025
What it means in practice. The first NK1/NK3 option becomes available in the United States. A better liver profile than fezolinetant, plus an added benefit on sleep.
www.fda.govFDA, December 2024
What it means in practice. A mandatory ALT/AST monitoring protocol, with a check before starting, monthly checks for the first two months, then at 3, 6, and 9 months. Stop immediately if ALT/AST is greater than 3x the upper limit of normal with bilirubin greater than 2x the upper limit.
www.fda.govFDA, May 2023
What it means in practice. The first targeted non-hormonal VMS option from a new class (a selective NK3 antagonist acting on hypothalamic KNDy neurons).
www.fda.govMHRA, July 2025
What it means in practice. The UK becomes the first market to offer elinzanetant. NHS access by prescription. France and Europe remain under EMA review post-2025.
www.gov.uk/government/organisations/medicines-and-healthcare-products-regulatory-agencyEMA, European approval
What it means in practice. Real-world access in France depends on national listing decisions. Enhanced liver monitoring aligned with the FDA and MHRA.
www.ema.europa.euPivotal clinical trials, from WHI to OASIS
The trials and cohorts that moved clinical practice. The scientific basis behind the guidelines and regulatory decisions above. For each one, the design, the year, the sample size when it is known, and the key takeaway we hold on to.
WHI, Women's Health Initiative
The 2024 Manson reanalysis reframed the original reading. The risks seen in 2002 were tied to the average age at initiation (63) and to the conjugated equine estrogens plus medroxyprogesterone regimen, not to HRT in general.
www.whi.orgKEEPS, Kronos Early Estrogen Prevention Study
The foundation of the timing hypothesis. HRT started soon after menopause in healthy women does not worsen the surface cardiovascular profile.
KEEPS-Cog Continuation Study
No lasting cognitive benefit, and no clear cognitive harm either. A mechanistic signal of lower brain amyloid plaque burden on transdermal estradiol vs placebo.
ELITE, Early versus Late Intervention Trial with Estradiol
Confirms the timing hypothesis for vascular health. Estradiol started early slows the progression of atherosclerosis, an effect that disappears when initiation is late.
www.nejm.orgSWAN, Study of Women's Health Across the Nation
A real-world map of the transition, the median duration of hot flashes (7-10 years depending on profile), metabolic and bone trajectories, and ethnic variation.
BMJ 2024, Swedish cohort using target-trial emulation
Risk varies by HRT regimen. An excess of about 11 cases of ischemic heart disease per 1,000 women per year with certain continuous combined oral forms and with tibolone. No clear cardiovascular signal for the transdermal routes.
www.bmj.comKotsopoulos et al., BRCA cohort
No increase in breast cancer with HRT in BRCA1/2 carriers after risk-reducing oophorectomy. In BRCA1 carriers on estrogen alone, a reduction in breast risk was observed.
jamanetwork.comManson et al., WHI reanalysis
The reference text for dismantling the binary HRT-equals-danger reading. HRT is neither a primary-prevention strategy nor off-limits. It is individualized.
jamanetwork.comLIFTMOR, Lifting Intervention for Training Muscle and Osteoporosis Rehabilitation
High-intensity Resistance and Impact Training is safe in women with osteoporosis when supervised, and is the most effective mechanical stimulus for lumbar bone mineral density.
LIFTMOR-M
Confirms the generalizability of the HiRIT protocol. Serves as a methodological point of comparison for the postmenopausal woman.
ONERO
The LIFTMOR protocol put into practice at clinical scale. The reference standard when the AI talks about supervised bone training.
OASIS 1, 2, and 3
A reduction of about 3.2 moderate to severe vasomotor episodes per day at 12 weeks vs placebo, at the 120 mg/day dose. A significant improvement in sleep. A better liver profile than fezolinetant.
SKYLIGHT 1 and 2
Significant efficacy on hot flashes from the very first day of treatment at 45 mg/day, versus placebo. The scientific basis for the FDA approval in May 2023.
SURMOUNT post-hoc Mayo Clinic, tirzepatide plus HRT
Additional weight loss on the order of 35% compared with tirzepatide alone, in postmenopausal women with obesity on the tirzepatide-plus-HRT combination. An observational signal, not a standard protocol.
Lancet Healthy Longevity 2025, HRT and dementia
HRT prescribed for hot flashes is not associated with any change in dementia risk. A relative risk of 1.16, with a confidence interval consistent with a trivial or null effect. Confirms that HRT is not prescribed to prevent dementia.
www.thelancet.comJAMA Oncology 2024, vaginal estrogen after breast cancer
No increase in cancer-specific mortality with the use of local vaginal estrogen after a breast-cancer diagnosis. An adjusted hazard ratio of 0.77. A pivot for the second-line GSM discussion.
jamanetwork.comChen et al. and the Gynexelle cohort, vaginal hyaluronic acid
Hyaluronic acid restores the target vaginal pH (3.8-4.5) and tissue hydration in about 4 weeks, with efficacy comparable to estrogen creams over the short term.
Basson, model of female sexual desire
The responsive-desire framework (vs spontaneous) that still shapes the clinical management of low libido in peri/menopause today. The reference for the Intimacy and mood section.
Validated scales and criteria
The validated instruments behind the quiz and its interpretation. Not one of them is homemade.
Greene Climacteric Scale
21 items, four subscales (psychological, somatic, vasomotor, sexual), scored 0-3 per item. This is the instrument that scores your Decodemeno quiz.
STRAW+10, Stages of Reproductive Aging Workshop
The international reference for staging the peri/menopause transition. Used to classify your phase (early peri, late peri, early post, late post) from your cycle profile and symptoms.
MENQOL, Menopause-Specific Quality of Life
A validated questionnaire for measuring the impact of symptoms on quality of life (vasomotor, psychosocial, physical, sexual). Used alongside Greene for follow-up.
PSQI, Pittsburgh Sleep Quality Index
The reference scale for sleep. Used by the AI when sleep-maintenance insomnia and fatigue are front and center, before pinning everything on hormones alone.
HADS, Hospital Anxiety and Depression Scale
Lets us quantify the anxiety-depression component of the transition, and guide how symptoms are read without confusing perimenopause with major depressive disorder.
Menopause Rating Scale, MRS
Cross-validation of the severity tiers and the quality-of-life framing. Especially useful for international comparison.
Reference books, the foundation
The eight books that form the backbone of the AI. Haver, Mosconi, Gunter, Bluming, Attia, Northrup (with guardrails), Galveston, Snowdon.
The New Menopause
The backbone of the symptom catalog (more than 47 documented symptoms), a timing-aware pro-HRT stance, and an anti-pseudoscience hierarchy.
The Galveston Diet
The nutrition and metabolism chapter, anti-inflammatory eating, how to frame intermittent fasting, and the metabolic shift of perimenopause.
The Menopause Brain
The neuroscience chapter. Estradiol as a central regulator of the brain's energy metabolism, brain fog, the Alzheimer's risk window, and the PhytoSERM.
The Menopause Manifesto
The evidence-based anchor. The hierarchy of evidence, an anti-pseudoscience stance, GSM as standard of care, and the menopausal hormone therapy vocabulary.
Estrogen Matters
A critical reanalysis of the WHI trial, HRT in breast-cancer survivors, and the Bradford Hill criteria applied to claims about HRT and cancer.
Outlive
The healthspan framing, the four major diseases of aging, ApoB and cardiometabolic risk, and the training quadrant (Zone 2, VO2max, strength, stability).
The Wisdom of Menopause, revised edition
A bio-psycho-social framing of perimenopause as a life turning point, micronized progesterone, and local vaginal estrogen. Used with explicit guardrails, the post-2018 anti-vaccine and energy-medicine positions are excluded from the knowledge base.
Just Getting Started
An editorial reference. The wise older sister tone, the lived experience of medical limbo, and the everyday language of the peri/menopause conversation.
Citation policy
A source or nothing. Every factual claim in your report is sourced from the knowledge base. If a piece of information is not backed by a study, a guideline, or a book on the list above, it is not in your report.
Easy to read. We never name an author in the body of the report. References are recalled at the end of each section, so they do not weigh down the reading. You read about your situation, not a scientific journal.
Institutional links only. The links you see on this page point only to the official website of the organization cited (NICE, FDA, EMA, MHRA, professional societies). When we do not have a verifiable canonical page for a specific clinical trial, we leave out the link rather than invent one.
Always up to date. When the science of the peri/menopause transition moves (for example the FDA removal of the HRT Black Box in November 2025, the arrival of elinzanetant at the MHRA in July 2025, or the new AHA/ACC 2026 targets for Lp(a) and ApoB), our knowledge base is updated in step. Your purchase covers future method revisions, so you can regenerate a report later at no extra cost.
Level of evidence preserved. An observational study is not a randomized trial. A signal is not a standard of care. The report keeps that nuance, and tells you when a topic sits in an area of ongoing debate.
You can write to us. If you think an important reference is missing, or you see a claim that feels off, we want to know. Write to us, we read everything.
Last reviewed, May 2026. Next audit, June 2026. The knowledge base is revised with every major guideline change or pivotal trial. Your report can be regenerated at any time to fold in these updates.
